3D! Community Support

3D! Community Support enables individuals access to flexible and personal support by
supporting individuals to develop a personal plan tailored to their individual requirements
and needs. The 3d! Community Support offers Person Centred Planning, Life Coaching and
Circles of Support.

The project helps to enrich the individual’s lifestyles by facilitating social networks and
opportunities, support with training and educational opportunities, advocacy and
developing links with your community. The project supports the individual to develop
life skills and experiences to enhance interdependence.
