Cardiff Breakthrough Project
The project offers structured support to children and young people with a learning disability aged 8-18 years, within Cardiff, whose needs are so complex that they are at risk of school exclusion, and possible family breakdown.
The project only offers a limited number of children /young people an opportunity to benefit from this regular intensive support.
Staff consistently develop and follow behaviour management plans, which are aimed at reducing levels of challenging behaviours, and increasing the likelihood of more appropriate behaviour. Staff support is usually on a 2:1 basis. Support and guidance, and clinical responsibility is managed by the Children’s Intensive Support Services for some of the young people accessing the service and the project also accesses community psychology support as is required.
