Child Health & Disability Team

The team has a Service Manager, Assistant Team Manager, 3 Social workers, Community Learning Disability Nurses, Paediatric Occupational Therapists, a Community Support Worker, a Transition Worker and input from Special Needs Health Visitor.

Some of the children/young people, who are referred to the Children and Disabilities Team, may have specialist needs that require input from other teams. For example some children will require specialist sensory impairment work via social workers for those with Sensory Impairments.

Disabled Children are defined as children/ young people (aged 0 – 18 years) who:

  • Have a learning disability with an IQ under 70
  • Have a significant physical or sensory impairment.

At 18, all children will be transferred to the Adult Teams. This transition process will be carefully managed to ensure that needs and services are still provided for.

All referrals in the first instance for other services e.g. psychology input, will be made to the team and then passed on internally.

If no - one is available, messages can be left on the answering machine and a Social Worker will get back to you.
