Support for the transition of children with additional needs into school

The Early Years Education Transition Worker supports parents of children who have additional needs to enable the smooth transition of children going into nursery or reception class in the Cardiff area.

The Early Years Education Transition worker works alongside the Early Years Special
Educational needs Co-ordinators for Cardiff and receives referrals from the Early Years Forum.
