Ty Hafan - The Family Hospice for Young Lives

Ty Hafan provides palliative care to children with life limiting conditions between the ages of birth and 19 years. Palliative care includes supporting the child and the family through all states of the disease process, including the provision of short break and end-of-life care and afterwards into bereavement.

Ty Hafan aims to improve the quality of life of children with life-limiting conditions, delivering a family centred service that values all members of the family. The child is supported in developing to their optimum potential through play and therapies provided by Ty Hafan.

In addition to short break and end-of-life care, Ty Hafan provides practical, emotional and
psychological care to all members of the family through the Family Support Team. A range
of other community support services are also available, including Outreach Play.

Short break care is provided at the hospice based in Sully. Planned stays are organised
with up to 10 children being accommodated at any one time. Parent accommodation is also available for those parents who wish to stay.

Support is provided to the family following the death of the child, including assistance with the planning of funerals and other practical help.

Support and education is also provided to all professionals involved with the child, including teachers, nurses and carers.
